If any of this sounds familiar, you're in the right place.
You're somewhere between 38 and 52, and something changed, you just can't name it.
Age 38-52
You've been offered an antidepressant for what you're fairly sure is a hormone problem
The mismatch
You've had labs run. Everything came back "normal." You still feel wrong.
The Labs
You've been told you're "too young" for this conversation.
The Dismissal
Sleep broke first. Then your mood. Then your body stopped responding to what always worked.
Sleep First
You want real information, not a supplement someone's selling you.

ABOUT THE BOOK
Treating the decade, not the symptoms.
Most books hand you symptoms one at a time. One chapter on hot flashes. Another on sleep. Another on weight.
But joint pain, brain fog, broken sleep, mood swings, and a metabolism that stopped cooperating aren't separate problems. They're one hormonal shift showing up in different systems, and treating them separately is why nothing has worked.
PIVOT connects them, then gives you the plan.
This book exists to close that gap.
The PIVOT Framework
Five moves that turn the transition
into a plan.
Every chapter follows the same structure, so you don't just understand what's
happening, you know what to do about it.
P
Prepare
What's actually happening in your body, and why this decade matters more than any other for how you age.
I
Intervene
The hormone therapy conversation you deserve. The labs that show you something real. Precision instead of guesswork.
I
Vitality
Nutrition that works for this decade, muscle-first exercise, sleep as medicine, and sex as a vital sign
O
Optimize
The overlooked levers, cortisol, stress, connection, and how to reassess what you actually need.
T
Transcend
Longevity, and the woman you're becoming.

DOUBLE BOARD CERTIFIED OB/GYN & INTEGRATIVE MEDICINE
For years, I was the doctor on the other side of the desk the one with twelve minutes, a full schedule, and training that barely touched this.
That's not an excuse. It's the system most of us practice in. So I went back and trained in integrative medicine, because I wanted the other tools nutrition, sleep, metabolic health, stress without giving up the evidence base I trained in. Not one or the other. Both, held to the same standard.
That's the book.

What readers are saying
Reading PIVOT was finding the information I'd been looking for since perimenopause started. I ended the book feeling encouraged about this chapter of my life and the one that follows.

FAQ
No. Hormone therapy is one chapter of five. The book covers nutrition, exercise, sleep, metabolic health, and non-hormonal medication options in equal depth. If you can't take hormones, or don't want to, most of PIVOT still applies.
No. The perimenopause chapters will explain what happened to you, and the sections on bone health, cardiovascular risk, metabolic health, and longevity are written for exactly where you are.
That's arguably the best time to read it. Most women spend years unsure. The first section is built to help you figure out where you are.
It's more useful with one, and the book is largely about how to get a better conversation out of the one you have. But the nutrition, exercise, sleep, and stress sections are things you can act on yourself.
No. There's nothing to purchase and no branded protocol. Where supplements have reasonable evidence, they're mentioned. Where they don't, that's said too.
Yes. Several early readers handed it to their partner before finishing it themselves.
