Most women know progesterone has something to do with pregnancy. But what most doctors don't tell you is that progesterone is one of the most powerful hormones in your body, affecting your sleep, your mood, your skin, your heart, and your menstrual cycle. And it starts declining years before you reach menopause.
If you've been feeling off lately and can't quite explain it, this post is for you. What is progesterone and why does it matter?
Progesterone is produced primarily by the ovaries after ovulation, during the second half of your menstrual cycle. Think of it as estrogen's natural counterbalance. Where estrogen is stimulating and growth-promoting, progesterone is calming, cooling, and regulating.
It acts on GABA receptors in the brain (yes, the same receptors anti-anxiety medications target), helps regulate your menstrual cycle, supports your uterine lining, promotes restful sleep, and plays a protective role in bone health, thyroid function, and even your cardiovascular system.
The critical thing to understand: progesterone is the first hormone to decline as you enter perimenopause, often 7 to 10 years before your last period. That means women as young as 35 to 38 can start experiencing low-progesterone symptoms while still having regular cycles. The 7 surprising signs
This is one of the most common and most overlooked signs. Progesterone works directly on the brain's GABA receptors, the same calming pathway that sleep medications target. When progesterone drops, you may find yourself lying awake, waking at 3 or 4am wide alert, or feeling like your sleep is shallow and unrestorative no matter how many hours you log.

Progesterone is your built-in chill pill. Its metabolites act as natural anti-anxiety agents in the brain. When levels fall, many women describe a new and pervasive anxiety, feeling on edge, overly reactive, or emotionally heightened in ways they've never experienced before.
The research backs this up: approximately 55% of perimenopausal women report anxiety symptoms, compared to their pre-perimenopausal baseline. Progesterone is produced only after ovulation. When you ovulate less consistently, which is exactly what happens in early perimenopause, you produce less progesterone. Less progesterone means the uterine lining isn't properly regulated. The result: shorter cycles, longer cycles, heavier bleeding, mid-cycle spotting, or a luteal phase that's too short to support a healthy cycle.

If PMS that was manageable in your 20s has become harder to handle in your 30s or 40s, this is a classic sign of declining progesterone. Progesterone's role in stabilizing mood in the second half of your cycle means that as levels drop, the week before your period can feel increasingly intense: more irritability, more fatigue, more bloating, more emotional volatility.
In severe cases, this becomes PMDD (premenstrual dysphoric disorder), a condition with well-established links to low or unstable progesterone. Many of the women I see have been told this is "just stress." It rarely is just stress.
Low progesterone allows estrogen to run relatively unchecked, a state sometimes called estrogen dominance. One of the effects of this imbalance is fluid retention and bloating. Women often notice this around the abdomen and describe it as "puffiness" or "feeling inflamed," separate from actual fat gain.
Progesterone also interacts with the serotonin system in the gut, which affects motility. When progesterone drops, constipation, gas, and bloating can worsen noticeably. When levels drop, many women notice their skin becomes suddenly dryer, thinner, or more prone to fine lines, even if they've had oily or combination skin for years. This accelerated skin change isn't vanity; it's a measurable biological shift.

How to test your progesterone levels
Timing is everything. Progesterone levels peak about 7 days after ovulation, so for a 28-day cycle that means around day 21. Testing outside this window can give a falsely low result that leads to a missed diagnosis.
Ask your provider for a serum progesterone test (sometimes listed as "PGSN"). A result below 10 ng/mL in the mid-luteal phase is generally considered suboptimal for most symptomatic women, though "normal" ranges can be misleadingly wide. Lifestyle and nutrition first
Chronic stress is one of the most underappreciated causes of low progesterone. Elevated cortisol directly suppresses progesterone production and can block progesterone receptors. Reducing stress, prioritizing sleep, and moderating high-intensity exercise can all support progesterone production naturally.
Certain nutrients also support progesterone: zinc, magnesium, vitamin B6, and vitamin C all play roles in the ovarian cycle that produces progesterone. Bioidentical progesterone (BHRT)
